Refund requests must be made in writing and forwarded to the Registrar. A $25.00 service fee will apply to all NSF cheques. Agreement of conditions to participate in East Nepean Little League: I, the parent or guardian of the above-mentioned dependant, give my permission to his / her participation in any and all activities of Little League during the 2012 season. In case of injury to my dependant, I hereby waive all claims against the organizer, sponsor, or any supervisor appointed by them. I shall furnish a birth certificate for the child upon request of league officials. I agree to return all uniforms and other equipment issued to my dependant in clean and good condition except for normal wear and tear.
